Diamond Award

• There are seven categories of nominees, Receptionist/Exit Clerk/Continuing Care Clerk, Benefits Coordinator/Specialty Coordinator, Dental Assistant/RDA/RDA-EF/Lead Assistant/Specialty Assistant, Hygienist, Associate Dentist/Owner Dentist, Office Manager/Regional, and Support

Platinum Award

• 1% of Total Employees

Gold Award

• 3% of Total Employees

Evaluation Process

Step 1 - Minimum Requirements

• Length of Service – 6 consecutive months
• Discipline – No disciplinary action in past 12 months
• Safety – No safety or performance violations in past 24 months
• Attendance – Meets established guidelines
• Most recent performance ratings – 3.5 or above

Nominations meeting the minimum requirements are forwarded to the appropriate Supervisor for additional evaluation. Team Members not meeting the minimum requirements above will be disqualified from consideration for the XP Award.

Step 2 - Supervisor Evaluation

• Supervisors review the nominator’s comments and have the opportunity to support the nomination and provide additional information.
• Supervisors indicate their recommendation on the evaluation form.

All qualified nominations including, those with a Supervisor “no” recommendation, are forwarded to the Selection Team for consideration.

Step 3 - Selection Process

• The Executive Team reviews the nominations for specific examples of “clearly outstanding” performance in the four nomination categories.
• Nominees selected to receive an award require the final approval of the Executive Team.

All XP Award Winners are invited to attend the XP Conference in the year that the award is given.
